The New York medical examiner has just released the official cause of death for actor Philip Seymour Hoffman, listing “acute mixed drug intoxication” as the reason he died suddenly on February 2, 2014 at the age of 46. Specifically, Hoffman was killed earlier this month by a deadly mixture of heroin and cocaine — commonly known as a speedball — and several other drugs, including benzodiazepines (anti-anxiety) and amphetamines (uppers).
